We provide support and oversight of end-of-life care services in the community and primary care across Wandsworth.
This involves working closely with practices, CLCH community nursing services, specialist providers like Royal Trinity Hospice and Marie Curie and acute palliative care services to ensure people receive the care they need as they reach the end of their lives.
We do this in a number of ways, including:
- Supporting practices provide end of life care, in particular ensuring awareness of the pathways available and enabling them to deliver anticipatory care planning. This involves ensuring that the wishes and care needs of patients and their families are understood and care plans are identified and recorded appropriately
- Reviewing feedback from patients and their families and a range of other stakeholders to review their experiences and facilitating change in service delivery where necessary
- Monitoring the outcomes and quality of service delivery
- Providing opportunities for services to share knowledge and learning to improve the provision of care and communication between services
- Facilitating learning opportunities and resources across the borough to support staff to deliver high quality care
- Working with partners across South West London, regionally, and nationally to ensure that services incorporates evidence-based and best practice guidance and meets the priorities for palliative and end of life care services.
